Root Cause Analysis

Root cause analysis (RCA) is a class of problem solving methods aimed at identifying the specific base causes of problems or events.

The practice of RCA is to best solve problems by attempting to correct or eliminate root causes, rather than addressing the symptoms. The outcome of the RCA are corrective measures to reduce the likelihood of a problem reoccurring.

RCA is often considered to be an iterative process, therefore a tool for continuous improvement.

Root cause analysis is not a single, sharply-defined methodology; there are many different tools, processes, and philosophies of RCA in existence. Some of these processes are:

Safety-based RCA relates to occupational safety and health.
Production-based RCA relates to quality control in manufacturing.
Process-based RCA relates to business and systems processes.
Failure-based RCA relates to the practice of failure analysis of components.
